Membership Change Request

Please note: All fees charged immediately. An upgrade requires a deposit of the future final month dues, equal to one month of membership. An upgrade in most cases also requires a payment of the difference in this month's prorate between the 2 membership types for the remaining days in this month. An upgrade also requires a key fob purchase of $10 per added member. If a person you are adding already owns their own Court House Family Fitness or CH Fit 24 fob, please note that in the comment section, along with their name. *Please note a family consists of those who reside in the same household, with a max of 2 adults included in the membership.

HOLD REQUESTS must include at least 30 days of hold time, and are offered currently as a courtesy and are not part of your membership agreement. The courtesy hold will automatically go into effect on the first day of next month, and will end after the date indicated. If anyone on your membership swipes their fob or checks in at a reader or the desk, the membership end will hold immediately for all members and is not reversible. If you would like to add a new hold you will need to fill out a new hold form. If you would like to extend an existing hold that has not ended, please fill out a new hold form before your existing hold end date. Max hold time per 12-month period is 90 days. Any exception to this policy does not negate the policy. I also understand that if I submit notice to cancel while a membership is on hold, that hold will end immediately, a prorate for the remainder of the month will be posted and charged immediately, my final billed month will be next month and next month will be my final month of access. If I am a Court House Fitness Dual-Club member, my prepaid final month deposit will be placed against next month. *

CANCELLATIONS OR DOWNGRADES: Cancellation of all or some members and downgrade of all or some memberships (a form of cancellation & any change that results in a lowered monthly cost) require a full calendar month written notice. Next month is the final billed month and the final month of access. In a downgrade situation, next month is the final billed month of the current membership, and the new membership will start on the first day after the final day of the current membership type. If a Court House Fitness Premier Dual-Club member, your prepaid final month dues will be applied against next month's fee. If not everyone on the membership is cancelling or downgrading, only a partial amount of the prepaid final month deposit will be applied against next month's fees. *

*At submission you should see a THANK YOU note and you should receive an EMAILED COPY of this form. If you do not, this means submission failed and you will need to submit again or email us instead at courthouse@abkfun.com to submit your written cancellation request.
